Important Instructions - Please Read - You Must Submit This Form to Complete Camp Enrollment

If you have reached this form, you have already paid the camp fees with a credit card or have elected to sign-up for camp using a postal money order or personal check.  Please fill out the following form, in its entirety, as it contains important information necessary to enroll campers into our program and process your payment.  Make sure you select which camp you have signed up for (by checking the box to the left of the camp date) and tell us the number of campers you wish to enroll (in the category titled "Number of Campers").  All other fields must be completed before you will be allowed to submit the sign-up form.  When you have finished filling out the information, please press the submit button below.  If you have any questions regarding this form, please email signup@teacheshoops.com.

If you elected to pay for tuition by credit card, please keep the PayPal confirmation e-mail that you received as proof of purchase.  Please bring this confirmation receipt with you to the first day of camp to help expedite sign-up. 

Payment by Check or Postal Money Order:
 Please make Checks and Postal Money Orders out to Teaches Basketball Camps, LLC and send to: 59 South Greeley Avenue, Chappaqua, New York 10514.  Please make sure you have written the camper's name on the check.

I understand that injuries are inherent with a sport activity.  In the event that an injury occurs to myself or a family member, I agree not to hold Teaches Basketball Camps LLC and the sponsor, and any person connected with Teaches Basketball Camp LLC or the sponsor liable for any and all claims for bodily injury and property damage arising from participation in Teaches Basketball program or camp.
